This spells it out pretty well.

It used to be a pooners review board expression (used by pooners exclusively, that is) that meant a lady with very few restrictions, not only full service, but many positions, multiple shots if you were up to it, definitely a bj (possibly uncovered), definitely dining at the Y (possibly at the O), possibly greek as well. If the lady took these things to the extreme, the lady would be called "high mileage". A GFE was a variation of this, meaning very friendly and "loving" as well, usually very doting on your comfort and pleasure.

Then the agencies and less scrupulous ladies got in on the act.

When you see these expressions in an ad now, they can mean anything from the above definitions to out and out rip-off. The agency quoted by Oldfart above gives a clearcut definition of the term. This is very, very rare (the definition) outside of this agency. Their definition is NOT the definition that a review board pooner of 10 years ago might have given. But c`est la vie...
