Sorry Zippy, you are mistaken. 011 is the international phone access code, telling your phone company you want an overseas number, it's called the exit code. Then comes the country code. Canada, the USA and parts of the Caribbean use 1. full list of worldwide country codes is here.
https://countrycode.org/ The next numbers for all country code "1" locations is the area code. Cayman Islands is 1-345 but 345 is the area code, just like 604 for BC/Lower Mainland or 416 for Ontario/Toronto.

If you were in LA, area code 212 and want to call a 604 number in Vancouver, it would be 1-604-123-4567. If you were in LA and want to call anywhere outside of the US, Canada and those few Caribbean places, it would be 011, then the country code, then the rest of the number as per the structure of that country. Some countries have area codes like we do, others have city codes, the numbers can be short or pretty damn long.
- For a complete example, say you are calling the British Museum in London, England in the United Kingdom. To call from the U.S., you use the IDD number "011." The country code for the UK is "44," and the area code for London is "20." The rest of the phone number is "7323 8299." So, in total, you would dial 011 44 20 7323 8299 to place your call to the museum.
So if our OP friend was in LA and dialed 011-604-123-4567 he is going to get connected (or not depending on his luck and the phone system there) to a number in Malaysia, which uses country code "60". It would actually be 011-60-4-567.
