Short version analysis: The helo did not maintain visual separation. The following questions are how, why, and what can be done to prevent a further tragedy.

Prevention is simple, more whiteness and more maleness. Isn't that the analysis I heard from noted aviation crash expert, Donald J. "Jumpseat" Trump, yesterday?

There should be no resson for military aircraft, or any other aircraft for that matter, to be in flightpaths of civilian aviation routes unless there is an intervention event. Proximity of two very different agendas in the same theatre is simply a bad idea. Manage the risks, not just the protocols, to reduce tragic events like this.

In industrialized workplaces, every day, we say that “Accidents don’t happen, Risky behaviours happen”.

The Pentagon, Federal Govt and a huge city all piled up make it unavoidable.
You can see Coast Guard helicopters and airplanes taking off and landing on the airport ramp from the gate window at Ronald Reagan.
It’s small but very busy.
Beats the hell hole of Baltimore.
Great views of DC when you land, recommend 10/10.

This army helicopter came from some base far upriver, and was going to somewhere downriver on a training flight, not to Reagan but to just do some urban environment training with NVGs on.
Apparently military helo fliights like this one are commonplace. It is not just mil aircraft using Reagan airport, but just coming and going from their own bases. (I saw an interview with a military helo pilot, who descrived flying with NVGs in such an area, and the sheer glare of lights making it hard to actually see individual objects clearly.)
And above DC, there's always fighters roaming around that area (since 9/11), but they keep out of the way of what goes on below unless they need to "intervene" for an incident.

I've heard it said that Reagan airport is one of those airports that would likely not be built today, considering where it is, with such busy & tight urban airspace. Probably the airport of choice for DC big shots, but other than them, it is really not a great place for major airlines to come and go.

(Funny fact: As a youth I once flew from that airport to NYC, on Trump Airlines. ...)

I heard the helicopter crew may have been wearing night vision goggles, dramatically limiting peripheral vision.

They twice took responsibility for visual separation when tower recognized their proximity. They were 100 feet or so too high for their corridor.

Sounds like multiple copter fuck ups.

Having flightpath crossing under a take off or landing path with less than 500 feet separation is absolute craziness. Gaining or losing a 100 ft in a 3620 shaft horsepower helicopter probably takes nothing if flown manually. The check pilot should not have attempted to go through there in the first place. It was only a training mission.

Heard that too. Also heard they may have mistaken a different airliner, following a few miles behind the one they hit, for the one they were supposed to maintain separation from.
Well, shit happens, but the problem is: there's no such thing as a "fender-bender" in a mid-air collision. That is why I think the people touting things like flying cars and aerial drone deliveries are absolute fools. Adding more air traffic can only make things worse.
