Probably more of a gross-out and humiliation factor than a health risk. From a communicable disease standpoint, it's much riskier to swallow cum than it is to have it sprayed all over your face. I've heard that if you get it in your eye it can sting like hell, but I can't say I've ever experienced it myself.

That leads to the other turn on for me which is, yes, the dominant aspect of it. To put it bluntly, the humiliation effect. It is such a different way for me to behave. The eye is a direct pathway for infections, and so is the nose. For example, you're more likely to pick up influenza from rubbing your eye after contact with the virus, than if it entered through your mouth. Human saliva offers more protection from pathogens than tears can, though I would not recommend CIM as a safe activity either.
