k i booked a appointment today at 5 40 with a SP and we had some fun. while i was doing her doggie, for like 1 min 40 or so i wanted to switch to missionary and the SP notice the condom had slipped off.im not sure even if it slipped off or it broke. i guess no lube was used as why it slipped. i didnt cum , but not sure if she did. im kind of scared if she might have stds or hiv.

should i tell and ask the SP , and ask if she has stds. . i dont want to be too direct and maybe she might get mad or sue or something.

i didnt ask her after the session since i wasnt thinking and this never happen before. i been to and visited a few Sps before.

what should i do. im kind of naive. should i go to hospital and tell them the whole story that i visited a SP and paid for it or should i just saw a girl i met , but hardly met. would the hospital tell the cops???

Hi joe 45

It is difficult to know if someone has an STI or HIV. Most people do not feel or show any symptoms.

It can take a bit of time for STI to show up in a test, this means that going for a test straight away will not be helpful. After a risk the minimum time to wait for a test is about 2 weeks. If you get any symptoms like discharge from the penis or it hurts when you pee come in straight away.

Here is a link on what tests to get and how long you need to wait.

Here is a link for STI clinics. You can also get the tests done at any doctor’s office.

It can certainly be stressful thinking about what happened. I usually advise people to wait 2 weeks before having a test but, if you are feeling stressed and want to talk to someone you are welcome to come in sooner. We would just do a follow up test later on.

If a condom slips or breaks and pregnancy is a concern, remember you can take emergency contraception (morning after pill) up to 5 days later. Currently in BC it is available from many drug stores without seeing a doctor.

Don’t worry about talking with health care providers. All your information is confidential by law. We are use to people telling us about drug use, sex work etc… From my current understanding we are only required to report if something involves a child or someone says they are going to kill someone.

There is medication available that can reduce your risk to becoming infected with HIV after you have been exposed to the virus. It is commonly referred to as post exposure prophylaxis (PEP) for HIV. Some important facts about PEP are;

• You need to take it as soon as possible, best to take it within two hours. You can also take it up to 72 hours but it is not as effective.

• Emergency departments usually have physicians experienced in dealing with this type of situation.

• In BC it is only funded for people who have been accidentally exposed e.g. nurses and needle stick injuries, someone who has been sexually assaulted etc. It is not funded for people exposed to HIV as part of their personal lives (e.g. consensual adult sex, or sharing drug injection equipment). However these medications can be acquired privately at a pharmacy with a prescription from a physician.

• I called the shoppers drug mart at 1125 Davie Street and spoke with one of the pharmacist. They usually have the medication in stock and they are open 24 hours. It would cost approximately $1200 for the 28 day treatment.

• There is a limited amount of scientific studies examining the effectiveness of PEP, but they believe that it can reduce you risk of acquiring HIV by 75%

• There are real risks from taking the medication so when determining whether to take PEP it is a balance of the weighing up the risk of the medication with the risk of becoming infected with HIV from the exposure.

• The actual risk for HIV can depend on many factors
- Do you know if they are HIV positive?
- What type of sex did you have?
- For how long?
- Are they on antiretrovirals medication for HIV?

• Would be worth considering PEP for a high risk exposure e.g. having unprotected vaginal intercourse with someone that you know is HIV positive.

I have included a link from the Canadian AIDS Treatment Information Exchange for additional information.
